Project Outreach provides case management services to people with mental illness who are homeless or at imminent risk to become homeless. It is also referred to as Somerset County PATH, which stands for Projects for Assistance in Transition from Homelessness.
The program staff work with Somerset County residents and will customize the program services to meet the needs of the individual. A service plan is developed to assist in linkages to mental health services, physical health services, community resources, and permanent housing. The program assists with applying for entitlements such as temporary housing, food stamps, welfare and SSI/SSDI. Once immediate mental health needs are stabilized, we help individuals apply for permanent housing and provide up to 6 months of case management services to assist in successfully maintaining that housing.
