Senior Centers Offer Free Case Management Services
Managed by the Office on Aging and Disability Services to Help Seniors Live Their Best Lives
Somerset County Freeholder Shanel Y. Robinson announces that beginning in July, all of the six county-operated senior centers will offer free on-site outreach services to residents age 60 and above and their caregivers. County-operated senior centers are conveniently located in Basking Ridge, Bridgewater, Hillsborough, Montgomery, Somerset and Warren.
Beginning in July, case managers will have a permanent monthly schedule at each senior center to welcome visitors that require assistance. Residents may walk in for assistance, or call to make an appointment, but an appointment is not necessary.
The role of case managers is to help residents remain independent and active in their communities. To achieve this, case managers use their expertise to evaluate residents, determine what programs and services are most appropriate for them and to provide referrals to those programs.

Case managers are also available from 8:30 a.m. to 4 p.m., Monday through Friday (except holidays), at the Office on Aging and Disability Services located at 27 Warren St., Somerville.
The Somerset County Office on Aging and Disability Services is the designated Aging and Disability Resource Connection (ADRC), which is designed to streamline consumer access to long-term support services, health promotion and education.
