MUNICIPAL PLANNING PARTNERSHIP GRANT APPLICATION FORM

All applications for Municipal Planning Partnership Grants shall address the following:

PROJECT DESCRIPTION:

Provide a brief description of the project.

GRANT REQUIREMENTS/REVIEW CRITERA:

• Purpose Statement: Describe how the project will further Inter-jurisdictional/Regional Cooperation, Smart Growth Concepts and/or Regional Planning Initiatives as related to the Inter-local agreement on “Projects of Regional Significance” and/or the NJ Smart Growth Planning Grant Program. Refer to the MPP Guidelines regarding “Eligible Projects” where applicable.

Please also address the following:

Demonstrate Readiness to Proceed:

Identify date at which project will begin. Indicate availability of required funding match. If work is to be completed by municipal staff, indicate personnel assigned, their relevant experience and confirm that they will have time available to complete the project within the required time frame. If using a consultant, identify and indicate current relationship (under general contract, under specific contract for the grant project, not currently under contract). If available, a contract copy should be included indicating the consultant's scope of work and timetable for completing the project.

Indicate Ability to Leverage Other Funding:

Identify ability to leverage funding from other public or private sources. If Municipal Planning Partnership funding is being requested in conjunction with a related New Jersey Department of Community Affairs Smart Growth Planning Grant, include a copy of this with the Somerset County Municipal Planning Partnership Grant application.

Indicate Prior Record of Performance and Ability to Complete Projects within the Grant Timeline:

Identify record of performance with respect to timeliness of completion of projects previously funded in whole or in part by Somerset County, such as prior Municipal Planning Partnership Grants, Economic Development Incentive Grants, etc. A brief progress report should be included indicating status of any currently incomplete Somerset County grant sponsored projects. Indicate schedule for current project, not exceeding 24 months with milestone dates and deliverables identified.


MAJOR WORK TASKS / SCOPE OF WORK:

• List the major work tasks and provide an itemized description of the major work items to be performed in each task. Also include a description of the specific deliverables along with a schedule and set of milestones to be met towards the completion of those deliverables.

• The identification of work tasks and deliverables shall be organized and presented in such a manner as to facilitate a checklist-type assessment of scope of work compliance as the project progresses.

• Each proposal shall contain a work task that will involve meeting with adjacent communities that may be affected as a result of the grant project.

PROJECT BUDGET:

• Description of the funding amount requested, its relationship to the overall project cost and description of any additional funds that will be used in this project, i.e. cash from general funds, other federal or state funds, in-kind services, etc. Provide a detail of project costs associated with the scope of work, including consultant proposal were applicable. Lump sum estimates without backup information detailing relationship to scope of work items will not be accepted.

PROJECT MANAGEMENT:

• Description of project management procedures used to complete the proposed project. A specific project manager shall be designated who will be responsible for processing payment requests and for the submission of all required grant materials. The project manager will also be responsible for issuing a brief quarterly progress report providing a project update. Project manager contact must include address, telephone number and extension, fax number and e-mail address where available.

• Upon submission of the final work product, the project manager shall include a checklist of all originally proposed scope of work items, which shall include specific reference(s) describing where in the product each of the identified scope of work items are addressed and/or, as appropriate, how such item was completed.

• Identify all other appropriate contacts, with address, telephone number and extension, fax and e-mail address information.

APPLICATION MATERIAL

• The application material shall include an acknowledgement of receipt of the 2007 Somerset County Municipal Planning Partnership Program Guidelines.

• The application shall be accompanied by an executed resolution adopted by the municipal governing body authorizing the undertaking of the proposed project if Somerset County funds are awarded.

• Application submission shall include ten (10) copies of the application and one original.
