INTRODUCTION TO LIVING WILL
| Just as we create estate plans for our eventual
demise, we also need to plan ahead for the possibility that we will
become sick and unable to make our own medical decisions. The Advanced
Directive & Living Will document enables you to specify your treatment
preferences and appoint your health care representative. This document
directs the care that you desire and appoints the representative you
have named to act for you only when you are prevented by either physical
or mental incapacity from making your own medical decisions.
Furthermore, it is necessary to give a copy of the completed form to
those who should know about your preferences, such as family members,
friends and doctors. Also, this document will be requested upon
admission to health care facilities; and, should you enter a hospital or
nursing home make sure that your directive is made part of your medical
records.
